i.          Serial Number of the List available in Covering letter is not matching with the serial number of enclosed information sheet of KVs.

ii.         The Printout of the information sheet of KVs is not readable.

iii.        The Scout/ Guide Information sheet of Form No. 18095, 34485, 34486, 34493 and 34492 is not found attached.

iv.                  Please find enclosed herewith the 4^th Copy of the Form No. 34485, 34486, 34493 and 34492 which received extra without direction of this office.
